There's nothing special to creating a sitemap. Just take a look at other sites that have one and do likewise.

Link to the sitemap from your homepage. You do this so that the search engines can easily find it, and assuming that you've made sure to link to all of the pages of content on your site, the engines will easily be able to find and index all of those pages.
